Connect previously incarcerated individuals to long-term interventions via a Restorative Justice Connector Program; provide connections to nutrition services, workforce development, housing, life skills, mental health and substance use treatment, and physical health; conduct needs assessment for participating individuals; evaluate program outcomes.
Second Chance Act
Provide comprehensive services to 18-24 years old that have been assessed as moderate-to-high-risk to recidivate; screen participants and identify their needs for further assessment; develop case plans; provide peer specialist services and mentoring; access to permanent supportive housing.
Peer mentors and project managers work with incarcerated individuals who qualify for post-conviction sentence modification to plan for re-entry and address mental health and substance use disorder challenges.
